Our live-in transitional care program helps clients transition from hospital to home, with a focus on establishing independence. It may also be transitioning from a home to a community.
Live-In Care Service Process
Needs Assessment
A Regional Account Manager from Life Force collaborates with the client and their family to guide everyone through the process of establishing live-in care services. The Needs Assessment identifies the physical and emotional needs of the client and address all concerns and expectations of establishing services.
Starting Live-in Care Services
The Regional Account Manager along with the assigned Home Health Aide will establish services, establish expectations, and address any concerns from the family our caregiver.
Initial Nurse Supervisor Assessment
The initial nurse assessment is completed by a Director of Nursing or a Nurse Supervisors from Life Force. Working with the client they develop a Plan of Care to identify the goals of our services and personal care needs. Any medical assessments or advice would come from client's Primary Care Physician or specialist.
Social Worker Assessment
Our social worker communicates and meets with clients, families, and caregivers to offer community resources, emotional support, and guidance through the progression of the aging of our clients.
Maintenance of Services
Through the course of our relationship there will be routine calls and visits from the assigned Nurse Supervisor and Social Worker. The service plan of our clients fluctuates based upon their needs.
